Required Immunization:

  • Age 18-59: MMR, Varicella, Hepatitis B, Tdap/Td, Polio, Influenza (Annually), Covid 2023-2024
  • Age 60-64: MMR, Varicella, Tdap/Td, Polio, Influenza (Annually), Covid 2023-2024
  • Age 65-older: Tdap/Td, Vaicella, Pneumovax, Polio, Influenza (Annually), Covid 2023-2024
